Live Healthy South Carolina


It is our pleasure to announce the launch of a state-wide collaborative process to develop Live Healthy South Carolina, South Carolina’s first state health assessment and state health improvement plan. The SC Department of Health and Environmental Control will work in partnership with the Alliance for a Healthier SC over the next year and a half to develop a comprehensive picture of health factors and health outcomes in South Carolina and a multi-year roadmap for continuing to improve the health of all South Carolinians.

Please help Live Healthy SC with the below surveys! Both take less than 5 minutes.

We are asking that one person from health care and human service organizations complete this survey.

We are seeking assistance from our partners to identify aggregated data sets to include in the state health assessment. Our preference is to include data available at both the state and county level.  Your information and data will help us understand what is important and how we can better serve the state.  Once all data is compiled, we will share the results as part of our state health assessment report.

We need asking that your organization help distribute this survey as many South Carolinians as possible.

We are aware that many Alliance member organizations regularly work with their communities, and we want as much local input as possible as part of the State Health Assessment. The results will be used to identify issues that can be addressed through collaborative planning and action. It should take 5-10 minutes to complete.

Workgroup Goals:


The workgroup will:


  • Assist in identifying and obtaining data that will focus attention on the need to promote actions that protect health, prevent disease, and advance health equity.

  • Foster linkages among statewide and community-based initiatives that affect health and well-being.

  • Champion the creation of a State Health Assessment that will guide the development of a State Health Improvement Plan – a call to action for South Carolinians to take data-driven, evidence-based steps to advance the health and well-being of all South Carolinians.

Workgroup Information:


The leaders serving on this workgroup bring valuable expertise from diverse sectors and can create important linkages to myriad activities occurring across the state that support population health improvement. Advancing health and well-being for all South Carolinians requires broad, multi-sector engagement to support policy, environmental, programmatic and infrastructure changes that lead to thriving communities.
