2019 17th Annual Chronic Disease Prevention Symposium

Theme: “Innovation in Action: Building Blocks for Comprehensive Chronic Care”
Come and learn how chronic diseases impacts every patient. Topics will include heart disease, diabetes self-management, diabetes prevention and the National Diabetes Prevention Program, mental health, and so much more. Registration information will be emailed in early to mid-January 2019. It will also be available at www.chronicdiseasesymposium.org.
Who Should Attend: Primary care physicians, doctors of osteopathy, physician assistants, nurse practitioners, and diabetes educators. The session will be equally relevant to nurses, pharmacists, registered dietitians, certified health education specialists and social workers.
