The Alliance works in collaboration with SC Department of Health and Environmental Control (SC DHEC) and partners around the Live Healthy priorities to inform health improvement and equity work happening in communities.

Together we are Live Healthy

The Alliance works to connect and convene partners around the Live Healthy South Carolina (LHSC) priorities to raise up health improvement and equity work happening in communities across South Carolina to showcase best-practices, lessons learned and promote collaborative connections that build trust. We use the LHSC priorities to communicate efficiently to align partner resources and build capacity that supports equitable health.

Call for Presenters

The Alliance invites you to share programs, projects, and best practices that aim to improve health outcomes in our state. A selected number of submissions will be given the opportunity to be a part of the Live Healthy Meeting on December 7th.  Live Healthy South Carolina is a collaborative process led by the Alliance for a Healthier South Carolina (Alliance) to systematically assess and advance the health of all South Carolinians. South Carolina’s first comprehensive state health assessment (SHA) and the state health improvement plan (SHIP) were created through this initiative.

Submit your “bright spots” by September 27th.

Focus Areas

Innovative Approaches that Address Health Disparities & Health Equity
Overview of a successful evidence-based program/project that reduces health disparities and improves health equity.

Emerging Issue
Highlight a solution/best practice that addresses a clinical issue or social determinant of health (social/economic factor) impacting health outcomes.

Collaboration
Highlight a state or community partnership/collaboration that has taken specific action to improve population health.

Call for Nominations

The Live Healthy SC Awards program recognizes those individuals and organizations that exemplify the highest levels of commitment to improving the health and wellbeing of communities they serve. Those who receive this recognition will serve as visible examples of how the LHSC blueprint can be effectively achieved in practice.

Award Categories

Dr. Rick Foster Leadership Award
An individual who has served as a champion for health and health equity at a local, regional or state level, and exemplify in action the key LHSC aims, priorities and goals are eligible for the Capstone Leader award.

Community Innovation Award
A community coalition or community-based organization who demonstrates active engagement in leading and guiding innovative work for community health improvement and the elimination of health disparities.

State Impact Award
A state level organization or coalition who demonstrates a commitment to support and take action under the LHSC plan, using a collaborative approach for state health improvement.
